زمینه و هدف: رفتارهای پرخطر در هر جامعه ­ای یکی از عمده­ ترین عامل تهدیدکننده نظام سلامت تلقی می­شود. میزان خطرپذیری نوجوانان نسبت به دیگر گروه­های سنی بالاتر است و گرایش بیشتری به این نوع رفتارها در آن­ها دیده می­شود. پژوهش حاضر باهدف شیوع ­شناسی مصرف سیگار، الکل و مواد مخدر در بین دانش ­آموزان نوجوان استان گیلان صورت گرفت.
مواد و روش­ها: این پژوهش به روش زمینه ­یابی انجام شد. جامعه آماری مطالعه حاضر شامل تعداد 182637 نفر از دانش­ آموزان دوره متوسطه استان گیلان در سال تحصیلی 1400-1399 بود که تعداد 384 نفر از آنان به روش خوشه ­ای چند مرحله­ ای به عنوان نمونه انتخاب و در مطالعه مشارکت کردند. برای جمع­ آوری داده­ های مورد نیاز از مقیاس خطرپذیری استفاده شد. برای تحلیل داده ­ها در سطح توصیفی از روش­های آماری فراوانی، نمودار و در سطح استنباطی از آزمون­های ناپارامتریک کروسکال والیس و یومن- وایت­نی استفاده شد. تمامی عملیات آماری به وسیله نرم­ افزار Spss16 تجزیه و تحلیل شد. سطح معناداری برای تمامی آزمون­ها 0/05 در نظر گرفته شد.
یافته ­ها: نتایج نشان داد رفتارهای پرخطر مصرف سیگار (34/96%)، مصرف مواد مخدر (17/31%) و الکل (62/30%) به ترتیب بیش‌ترین شیوع را در بین دانش­ آموزان نوجوان داشت. همچنین نتایج نشان داد شیوع رفتارهای پرخطر در بین دانش ­آموزان نوجوان بر اساس سن و جنسیت تفاوت معناداری نداشت (0/05<p).
نتیجه ­گیری: مصرف سیگار، الکل و مواد مخدر در بین دانش ­آموزان نوجوان شیوع نسبتاً بالایی دارد که موجب بروز نگرانی ­هایی در زمینه سلامت اجتماعی می‌شود  و برای رفع این مشکل باید از تمامی ظرفیت­های موجود در جامعه استفاده کرد.
