fa چک‌لیست جراحی ایمن: مستندسازی دستگاه‌های الکتروکوتر و تورنیکت تخصصي Special پژوهشي Research <div style="text-align: justify;"><strong><span style="font-family:b zar;"><span style="font-size:14.0pt;">زمینه و هدف</span></span></strong>: <span style="font-family:b zar;"><span style="font-size:14.0pt;">مستندسازی مراقبت‌ها، از وظایف پرستاران اتاق عمل است. از مواردی که حین عمل می&shy;تواند سبب آسیب به بیماران شود و نیاز به ثبت دارد، استفاده از دستگاه‌های الکتروکوتر و تورنیکت است که جای آن در چک‌لیست‌های جراحی خالی است. لذا هدف از پژوهش حاضر، تغییر در چک‌لیست جراحی ایمن از طریق مستندسازی استفاده از دستگاه‌های الکتروکوتر و تورنیکت است.</span></span><br> <strong><span style="font-family:b zar;"><span style="font-size:14.0pt;">مواد و روش &shy;ها: </span></span></strong><span style="font-family:b zar;"><span style="font-size:14.0pt;">این مطالعه اقدام پژوهی، در سال 1396 بر روی 30 نفر از پرستاران اتاق عمل بیمارستانی در شهر اصفهان انجام شد. راهکار منتخب، افزودن چک‌لیست محقق ساخته به پرونده&shy;های اتاق عمل جهت ثبت استفاده از الکتروکوتر و تورنیکت بود که به مدت 3 ماه اجرا گردید. قبل و بعد از مداخله، یک فرم نظرسنجی محقق ساخته در خصوص وضعیت ثبت پرونده&shy;های اتاق عمل توسط نمونه&shy; ها تکمیل شد. </span></span><span style="font-family:b zar;"><span style="font-size:14.0pt;">آنالیز آماری با آمار توصیفی و تحلیلی در نرم‌افزار </span></span><span dir="LTR"><span style="font-family:times new roman,serif;"><span style="font-size:14.0pt;">SPSS 22.0</span></span></span><span style="font-family:b zar;"><span style="font-size:14.0pt;"> انجام شد.</span></span><br> <strong><span style="font-family:b zar;"><span style="font-size:14.0pt;">یافته‌ها: </span></span></strong><span style="font-family:b zar;"><span style="font-size:14.0pt;">از 30 نفر نمونه، </span></span><span style="font-family:b zar;"><span style="font-size:14.0pt;">رشته تحصیلی 28 نفر اتاق عمل با میانگین سابقه شغلی 8/30</span></span><span style="font-family:times new roman,serif;"><span style="font-size:14.0pt;">&plusmn;</span></span><span style="font-family:b zar;"><span style="font-size:14.0pt;">11/87 سال بود. فراوانی پاسخ مثبت به سؤال &laquo;آیا چک‌لیست جدید را کاربردی می&shy;دانید؟&raquo;، بعد از مداخله به‌طور معناداری بیشتر از قبل از مداخله بود (0/001</span></span><span dir="LTR"><span style="font-family:times new roman,serif;"><span style="font-size:14.0pt;">P<</span></span></span><span style="font-family:b zar;"><span style="font-size:14.0pt;">). 90% از پرستاران با افزودن چک‌لیست و 96/7% با تأثیر چک‌لیست جدید بر کاهش بروز خطای شغلی و آسیب به بیماران موافق بودند.</span></span><br> <strong><span style="font-family:b zar;"><span style="font-size:14.0pt;">نتیجه&shy; گیری: </span></span></strong><span style="font-family:b zar;"><span style="font-size:14.0pt;">چک‌لیست ثبت استفاده از الکتروکوتر و تورنیکت کاربردی بود. مستندسازی کامل، می&shy;تواند احتمال ایجاد خطا را کاهش دهد و سندی معتبر در مقابل مشکلات قانونی برای مراقبین درمانی فراهم نماید.</span></span></div> <div style="text-align: justify;"><strong>Background & Aim:</strong> Documentation of <gwmw class="ginger-module-highlighter-mistake-type-3" id="gwmw-15618781828923362523998">cares</gwmw> is one of the responsibilities of operating room nurses. One of the possible patient damages caused during surgery is <gwmw class="ginger-module-highlighter-mistake-type-3" id="gwmw-15618781840045299638908">use</gwmw> of <gwmw class="ginger-module-highlighter-mistake-type-1" id="gwmw-15618781840043558153247">electrocautery</gwmw> and tourniquet devices. Despite the need for recording the performance of these devices, there <gwmw class="ginger-module-highlighter-mistake-type-3" id="gwmw-15618781849206429175962">is</gwmw> no specific part on surgery checklists dedicated to this issue. Therefore, this study aimed to change the safe surgery checklist through documenting the application of <gwmw class="ginger-module-highlighter-mistake-type-1" id="gwmw-15618781856147060557182">electrocautery</gwmw> and tourniquet devices.<br> <strong>Materials <span dir="RTL">&</span> Methods:</strong> This action research was conducted on 30 operating room nurses in Isfahan, Iran in 2017. The solution of choice was adding a researcher-made checklist <gwmw class="ginger-module-highlighter-mistake-type-3" id="gwmw-15618781875648175726535">to</gwmw> the operating room files in order to document the use of <gwmw class="ginger-module-highlighter-mistake-type-1" id="gwmw-15618781875643259569557">electrocautery</gwmw> and tourniquet devices for three months. In addition, a researcher-made survey on the status of operating room file documentation was filled by the subjects before and after the intervention. Moreover, data analysis was performed in SPSS version 22 using descriptive and <gwmw class="ginger-module-highlighter-mistake-type-3" id="gwmw-15618781891277137194328">analytical</gwmw> statistics.<br> <strong><gwmw class="ginger-module-highlighter-mistake-type-3" id="gwmw-15618781900762883093473">Results</gwmw>:</strong> In this research, 28 out of 30 participants had a mean work experience of 11.87&plusmn;8.30 years in operating rooms. Furthermore, there was a significantly higher frequency of positive response to the question of &ldquo;Do you think that the checklist is useful?&rdquo; after the intervention, compared to before the intervention (P<0.001). In total, 90% of the nurses agreed with adding the checklist to patient records, and 96.7% confirmed the effectiveness of the new checklist on decreased professional errors and patient damage.<br> <strong>Conclusion:</strong> According to the results of the study, the checklist of use of <gwmw class="ginger-module-highlighter-mistake-type-1" id="gwmw-15618781928037863083203">electrocautery</gwmw> and tourniquet devices was useful.
